Contacting Me:
- I am available by land-line phone (6 minute recording time for each message)
- Fax
- Text
- For emergencies call 911.
Payment Options:
Patients are responsible for payment in full at the time of service. I accept personal checks and cash. Should your check be returned for insufficient funds, I will ask you to reimburse me for fees that my bank charged.
Insurance:
I do not participate with any insurance companies. I am considered an “out of network provider.” I will provide you with a complete “super bill” which will contain all the information necessary for submission to your insurance company. If you are not sure whether a service is covered by your insurance, I encourage you to call your carrier for clarification in advance. If you have a medical savings account or HSA, my office visits are traditional medical services and considered reimbursable expenses.
Missed Appointments:
Because my schedule is so full, I ask that you give me as much notice as possible if you need to reschedule or cancel an appointment. I set a minimum of 24 hour notice of cancellation. .If you cannot keep a scheduled appointment or give 24 hours notice of cancellation, you will be charged your regular session fee. Of course, I am very understanding in cases of an emergency and do not charge. One option for late notice appointment conflicts (babysitter cancelled, schedule conflict) is to have our appointment over the phone. Please check with your insurance company to see if this type of session would be reimbursed. Some people prefer to take advantage of the telephone appointment option.
Rates:
For CPT codes and rate information, please call my office at (607) 754-3646.
Being An Active Patient:
You must have an office visit of thirty minutes (my shortest visit) a minimum of once every six months to be in my active practice. There may be situations where you will require more frequent appointments either due to a change in your clinical condition or a change in medication.
